Privacy for appointment requests.
When you submit the booking form, Glow Match Texas receives the contact details and general treatment interest you provide. The request is stored in our Supabase lead table, and a Telegram notification alerts us so we can follow up.
What we collect
The form asks for your name, email, phone, area, treatment interest, and optional general question. It records the page where the request began and the time you gave consent. Please do not send health history, diagnoses, medications, photos, or other sensitive medical information through this form.
How we use it
We use your details to contact you about provider availability and the appointment location. We may share the request with the treatment provider for scheduling. The provider handles medical intake, consent, care, and aftercare separately. A submission does not confirm an appointment.
Where it goes
Form data is sent over HTTPS to our Supabase project and stored in a private lead table. A Telegram bot sends a notification to our lead inbox. These services process the details needed for scheduling. The public website does not expose the database service key or Telegram bot token.
Your choices
You can ask about your request or ask us to delete the contact information we control by emailing joseph.girges@gmail.com. We have not specified a fixed retention period; please contact us for the current handling of a particular request. Your email or phone provider may keep its own communications records.
